JOB SUMMARY:
Self-Direct, Inc. NYS DOH Licensed Homecare agency provides services to individuals who have survived
a traumatic brain injury, elderly, MS, Spinal Cord Injury, Alzheimer’s, and Dementia. We are currently
looking to fill positions available in the Madison County Region. Personal Care Aide provides assistance,
training and supervision with daily living skills/ tasks, personal hygiene, dressing, meal preparation, eating,
light household tasks, laundry, shopping, socialization, transportation and other similar type tasks. Personal
Care Aide will work in community and in homes.

JOB REQUIREMENTS:
A High School Diploma or GED is required. Current CNA, HHA or PCA certification is strongly preferred.
We will provide free PCA training to those not currently prequalified under a CNA, HHA, or PCA. A
background check, physical exam, and immunization records are required.

HOURS:
Hours vary, overtime may be required. Full-time, Part-time positions available, flexible schedule as
determined by supervisor.
WAGES:
Pay rate is $9.25 per hour.

BENEFITS:
Paid vacation and sick time.
