CERTIFIED HOME HEALTH AIDE – JOB ORDER NUMBER – NY1091228


JOB SUMMARY: The HHA is responsible for performing personal care and household delegated tasks for

home health/hospice patients and providing limited nursing care and therapy services to the patients under

the clinical supervision of a registered nurse and may receive guidance from other health care professionals

as outlined in the Patient Care Plan. Under direction may take vital signs, change dressings, assisting with

taking oral medications, assist patient with personal care: bathing, moving in and out of bed, using medical

equipment, assist with meal preparations and/or feeding, changing linen and performing light housekeeping

Provide accurate, legible and timely clinical documentation according to Agency guidelines, take prompt

action in response to changes in patient health condition. Notifying nurse in timely manner. Effectively

communicates with patient, caregivers, RN, case manager, and other managers.


JOB REQUIREMENTS: Valid NYS Home Health Aide certification. Recent graduates welcome or

evidence of work experience at Licensed Home Care Agency within the past two years. Recent Physical

Examination including 2 step PPD or chest x-ray results, MMR immunization, Varicella. Must pass

background check. Evidence of Employment Eligibility.


LOCATION: Metro NYC (5 boroughs & Nassau).


HOURS: Sunday through Saturday Hours: Varies; Hourly, Live-In, and Shift work
