JOB SUMMARY: 
ElderChoice, a Licensed Home Care Agency, is currently seeking professional, compassionate and reliable 
Personal Care Aides (PCAs) to provide in-home and community based services to persons with Traumatic 
Brain Injury (TBI), the elderly, and individuals with disabilities. 

JOB REQUIREMENTS: 
PCA, CNA, or HHA certification a plus, but employer will train qualified candidates. Please include your 
certificate number when applying so the employer may verify your certification on the DOH Home Care 
Registry. Minimum GED or HS Diploma required. All candidates must have reliable transportation and 
must meet all requirements. DOH also requires all staff to have proof of a physical within 1 year, and 
certain vaccinations, including an annual PPD, MMR etc. Reference/ security check and physical exam. 

HOURS: Full and part-time shifts available; must be able to work weekends, evenings and overnights. 

WAGES: Positions start at $9.50 per hour. ElderChoice offers a $300 Sign-On Bonus for pre-certified 
applicants. 

BENEFITS: Vacation, sick leave, holiday pay, retirement/ pension plan.

St., Herkimer on Tuesday, April 1, 2014, at 9:00 a.m. and 1:00 p.m. Please bring a copy of your certificate 
with you. Candidates must be there at the start of either session for a 15 minute PowerPoint presentation of 
an overview and requirements. Interviews will be conducted at the Employment Information Session. Space
